The Role
As a Support Coordinator, you will play a crucial role in assisting clients to develop, implement, and monitor their NDIS plans. You will work closely with clients to understand their needs, coordinate a broad network of supports, and empower them to make informed, independent decisions. Your expertise in Psychosocial Recovery Coaching and Level 3 Support Coordination will be vital in ensuring clients receive effective, tailored support services, ultimately enhancing their overall quality of life.
While your primary focus will be on Support Coordination, there may be occasional responsibilities involving direct client support, such as assisting with daily living activities and providing companionship. This ensures a holistic approach to care and maintains a strong connection with the clients you are coordinating services for.
Key Responsibilities
- Support Coordination: Develop and maintain strong relationships with clients, assisting them in achieving their goals by effectively coordinating their NDIS plans and supports.
- Plan Development & Implementation: Help clients develop and implement their NDIS plans, ensuring they understand their funding and how to access appropriate services.
- Service Coordination: Collaborate with healthcare professionals, community organisations, and other service providers to deliver optimal outcomes for clients.
- Psychosocial Recovery Coaching (PRC): Utilise your skills in PRC to support clients in overcoming challenges and achieving personal recovery goals.
- Level 3 Support Coordination: Provide high-level support coordination to ensure clients receive comprehensive, effective services tailored to their individual needs.
- Monitoring & Reporting: Regularly monitor client progress, assess the effectiveness of services, and report on outcomes to maintain quality and compliance.
- Crisis Intervention: Provide crisis support as needed, ensuring client safety and well-being.
- Client Engagement: Engage directly with clients to support their daily activities, including personal care, meal preparation, and social participation, as required. Assist clients in attending appointments, participating in social outings, and accessing community resources.
- Occasional Direct Support: Provide hands-on support to clients when required, including assisting with daily activities and encouraging social participation.
Skills and Knowledge
- Strong communication and interpersonal skills
- Proficiency in client management and rostering systems
- Excellent organisational and problem-solving abilities
- In-depth understanding of the NDIS and relevant legislation
- Empathy and cultural awareness when working with diverse populations
Qualifications
- Cert IV in Disability, Community Services, or Mental Health (essential)
- Bachelor’s degree in social work, psychology, community services, or a related field (desirable)
- Minimum 2 years of experience in Support Coordination within the NDIS environment
- Experience in Psychosocial Recovery Coaching and Level 3 Support Coordination
